Brydges Family Dental (“we”, “us”), 22270 Mill Rd, Mount Brydges, Ontario N0L 1W0, respects your privacy and protects your personal health information as required by Ontario’s Personal Health Information Protection Act, 2004 (PHIPA) and other applicable privacy laws. This policy explains what we collect through this website and at our office, why, and the choices you have.
Information we collect
- Contact and booking requests: your name, email address, phone number (optional) and the message you choose to send when you use our contact form, call or email us.
- Patient records: information we create and keep when we provide care, as required by the Royal College of Dental Surgeons of Ontario and other regulations.
- Website use: basic technical information such as your IP address, browser type and the pages you visit, recorded by our web server for security and to keep the site working, and by Google Analytics as described below.
How we use your information
- To respond to your enquiries and schedule appointments.
- To assess, plan and provide dental care, and to communicate with other health care providers involved in your care.
- To process payments and insurance claims, including claims under the Canadian Dental Care Plan, with your consent.
- To meet our legal, regulatory and professional obligations.
- To understand how visitors use this website so we can improve it, and to keep it secure.
We do not sell your personal information.

By submitting a form or contacting us, you consent to us collecting and using your information for the purposes above. You can withdraw your consent at any time, subject to legal or professional requirements, by contacting us (see below). Withdrawing consent may limit the care or services we can provide.
Service providers
We use trusted service providers to run this website and our communications. They may process your information on our behalf, and some may store it outside Canada (for example in the United States), where it may be subject to the laws of that country. These providers include:
- Contact form: our website form is provided by Fillout. What you submit is stored on Fillout’s systems and sent to our office; it is not stored on this website.
- Email: Microsoft 365.
- Website hosting: our web host in North America.
Cookies, analytics and third-party content
- This website uses cookies needed for it to work and for security.
- We use Google Analytics and Google Tag Manager to understand how visitors use the site (for example which pages are viewed). Google sets cookies for this and receives information such as your IP address and browser details. You can opt out with Google’s browser add-on (tools.google.com/dlpage/gaoptout) or your browser’s cookie settings.
- Our Google reviews are shown with a Trustindex widget, which loads from Trustindex’s servers.
- Our contact form loads from Fillout’s servers once you start using the page, and Fillout may set its own cookies.
- Maps load from Google only when you click “Show map”. When they load, Google may set cookies under its own privacy policy.
How we protect your information
We use reasonable physical, organizational and technical safeguards to protect your information, including encrypted (HTTPS) connections to this website, access limited to staff who need it, and secure storage of patient records. No method of transmission over the internet is completely secure, so please do not send sensitive health information through the contact form or by regular email.
How long we keep information
We keep patient records for as long as required by the Royal College of Dental Surgeons of Ontario and other applicable laws. Enquiries that do not lead to care are kept only as long as needed to respond and then deleted.
Your rights
You may ask to see or get a copy of your personal health information, ask us to correct it, or ask questions about how we handle it. Contact us using the details below. If you are not satisfied with our response, you may contact the Information and Privacy Commissioner of Ontario (www.ipc.on.ca).
